report of a specific individual's strengths and weaknesses relative to reading, math and writing tasks. Student: Jayden S. Grade: 4 Background Jayden S. is a 9-year old boy currently in the 3rd grade at Davenport Elementary school, MD. Jayden was referred to The IEP Team as an initial referral. The Team in Texas indicated they were in the process of assessing him, however, Jayden's family moved back to Maryland. Jayden lives with his father, stepmother and sister. His stepmother does not speak English and Jayden is reportedly bilingual. Jayden's father indicated he was diagnosed with ADHD and is currently on medication. His father stated it takes Jayden a few hours to complete his reading homework but he seems to enjoy completing his math homework. Jayden's teachers report he is very distracted and active. He has difficulty with completing assignments in reading and math. Reading is reportedly a challenge for him and he frequently loses his assignments. His writing appears to be large and spelling is a concern.
