The Piper Alpha disaster took place on an oil and gas platform in the North Sea and the Texas City refinery explosion took place within a petrochemicals process near Texas City. The facilities were operated by different companies, but the incidents had much in common. Consider the Piper Alpha and Texas City disasters and identify any common causes that contributed to both disasters.
