The report will contain: - A brief discussion of the incident (less than 1 page). [10 marks] Calculations to determine the TNT equivalence of the resulting blast and comparison with the actual inquiry figures. The report should present an aerial view of the plant delineated with the appropriate blast zones. Calculations supporting the construction of the blast zone contours and a critic of the contents of each blast zone should be provided. [ 30 marks] The student is expected to critically evaluate the safety management practice used by BP against the HSE prescribed safety management system [ ISO 45001 health and safety management standard] for the case study. The student is expected to lay out evidence-based conclusions when highlighting agreement/alignment and differences between BP's actual operating practice, procedures, plant design etc., and that prescribed by ISO 45001 [ 40 marks] Draw conclusions at the end of the report about the incident and how it could have been avoided including a discussion of how internal and external whistle blowing could have helped to prevent this incident.
