Problem Based Report KM6623 Industrial Safety, Health and Environment A. The report The formatting: The report must be written in the following format: 1. 2. 3. 7. The title of the report, your name, student registration number. Font: Arial, font size: 11 1.5 line spacing 4. Single column 5. References: UKM style (refer "The UKM Thesis Writing Guideline”) 6. 4000-5000 words (the word count refers to the write-up (including abstract) only, excluding captions for Figures and Tables, List of References, and any write-up not associated with the content/discussion). You MUST state the number of words at the end of the Conclusion section of the write-up. This is an INDIVIDUAL project: Write a report based on an event of accident/near miss/safety issues encountered in your company. The report shall cover ALL of the following items: 1. Introduction of the organization, the organization structure, and activities of the company 2. Safety committee ((including Safety officer)-How many people and who are they. 3. Law/ OSHA followed by the company 4. Hazard management: identification, risk assessment and risk control 5. The problems/issues: accident/near miss/safety issues. Explain the problem/issues 6. Accident near miss/issues; Formation of Investigation group (Who are they?), Investigation process; including legal action taken (if any) 7. Lesson learnt 8. Comments regarding the safety process/investigation/ prevention activities/other issues 9. Conclusion The report SHALL at least contain the following sections: 1. The title of the report 2. Abstract 3. Introduction (answering the following, but not limited to) a. What is the report about the organization's safety b. The background and its current state: Law/OSHA/ Hazard management C. The problems/issues: accident/near miss/safety issues and investigation process 4. Discussion: Lesson learned/ prevention/ comments 5. Conclusion 6. List of References (at least 10 articles from the SCIE journals) Use sub-sections to assist with the write-up. Submit your work in both PDF and Microsoft Word formats via UKMFOLIO (file size of not more than 10 MB each). A report on the similarity index (usually using Turnitin, a web-based plagiarism prevention system) MUST be attached at the end of the manuscript. The similarity index must not be more than 30%. B. Oral Presentation You will be given 15 mins (10 mins presentation, 5 mins Question and Answer) to present your work. You will be assessed based on the followings (marks: 10%*) 1. The elaboration on the issues and the main outcomes of the discussion. 2. The presentation flow (easy to follow and understand, creative and attractive) 3. Delivery (written and oral communications - clear written sentences, fluent voice and showing great enthusiasm when presenting. Avoid reading the texts when presenting). C.
