Student Assessment Case Study Directions: A Student Assessment Case Study is necessary for the Implementation and Evaluation of Instruction assignment. Choose a student whose special instructional needs are associated with

unique learning characteristics. The student would exhibit difficulties in reading, math, writing, and/or study skills. This should be a student with whom you can work for the duration of this multi-week project. In this assignment you will address Part 1 and Part 4 of the Implementation Plan. Part 1: Student Information and Two IEP Goals The purpose of Part 1 is to provide context for the student and intervention. Provide a summary of relevant information about your students and two goals for your target student. Present Level includes: ■ Specific assessment data (including baseline data) which link the student's current ability and functioning to the need for IEP goals and further intervention. The strengths of the child. Concerns of the parent/guardian for enhancing the education of the student. IEP Goals: Two IEP Goals (MUST address reading, writing, math, or study skills area) From your examination of the data, identify two academic concerns (i.e., reading, written expression, and/or mathematics). This statement needs be drafted as a narrative based on the synthesis of the data above. In other words, this statement needs to be very specific and provide logical cues for the student's goals. ■ Name, Grade/Age/Eligibility and/or Disability How the child's disability affects his/her involvement and progress in the general education curriculum; or for preschool children, participation in age-appropriate activities. ■ Part 4: Progress Monitoring Plan (1-2 paragraph description + data collection devices) Progress should be monitored for the overall goal written for students. Data (e.g., qualitative data, student attitude, engagement) should be collected to monitor progress toward meeting the other objectives written for students. Data should be presented graphically and interpreted with a narrative. Describe your plan to monitor student progress on the goals. How would you monitor student progress on the goals? What would you measure? The measure needs to use systematic tools (e.g., curriculum-based assessment, frequency check). You will have four lessons, so you need to monitor progress four times. Attach the data collection table. Since you have two goals, you need to attach two data collection devices which match your IEP goals. Reflection (1-2 paragraphs)—This is not the same reflection as you will complete in Week 6 Reflect on the development of your Case Study. What did you learn from investigating the multiple sources of data? What sources did you use to determine the student's IEP goals? Are there any skill areas of your own that you would like to develop further? Describe your decision- making process for developing the data collection plan. What did you learn from the development of Part 1 & Part 4 of your plan? Reflection must be connected to the course content and learning materials, citing using APA style.
