We all have someone we know who lives with a long-term health condition, and we can
see its impact on their daily life. In your own words, describe someone you know who lives with
a health challenge or clinical condition. You can write about a personal experience; the
experience of a family member or friend; or the experience of a client from your work or
volunteer activities.
Your brief essay should include the following information:
• Your name and the date.
• Age and gender of the individual affected; how you know them.
• Describe the life role(s) of this individual-parent, child, student, worker, patient, etc.
• Name of the condition this can be medical, developmental, psychiatric, congenital, etc.
• How did/does the condition affect the individual did/does the person experience
challenges to mobility, endurance, cognitive understanding, sensory systems, etc.? In what
ways?
• In what ways did/does the individual need to take care of their health/condition? Consider
special diet, exercise, medications, therapy, assistive devices, alternative techniques, etc.
• What daily life activities have been affected by this condition for the individual? Consider
self-care, sleep, school, work, home maintenance, community engagement, social, leisure,
play any area or specific activity that has become a challenge for the individual because
of a particular clinical condition.
• How did/does the condition affect the family are there challenges to caring for the
individual? Did/Does the person have certain tasks/life roles that need to be taken on by
other members of the family?
