Piper Alpha disaster (1976) and the Texas City refinery explosion (2005) were reviewed on the course. The Piper Alpha disaster took place on an oil and gas platform in the North Sea and the Texas City refinery explosion took place within a petrochemicals process near Texas City. The facilities were operated by different companies, but the incidents had much in common. Consider the Piper Alpha and Texas City disasters and identify any common causes that contributed to both disasters. In your own words, describe how these deficiencies contributed to the original disasters and critically discuss how such deficiencies might be prevented and/or mitigated against to prevent similar disasters occurring in other similar installations.
