From the following geriatric syndromes listed below, develop your own case study. a) Depression and Dementia b) Hearing loss and Visual impairment c) Malnutrition and Adverse Medication Effects d) Elder abuse and Pain e) Falls and Sleep disturbances f) Urinary incontinence and Pressure injuries Consider the many challenges of older adults and their family caregivers within a specific setting of the healthcare organization such as the emergency department, inpatient, outpatient (home or community setting), or during the transitions of care from the hospital to post-acute care and community/home. 2. Choose an appropriate Geriatric Model of Care based on your developed case study (e.g., Program for All Inclusive Care for the Elderly [PACE], Nurses Improving Care to Older Adults [NICHE], Acute Care for Elders (ACE), Guided Care, UCLA Alzheimer's and Dementia Care, Home-Based Primary Care (HBPC), and Transitional Care Model). Feel free to use other Geriatric Models of Care from your research. Explain the model and how it fits best in your own case study. 3. Provide an example of an evidence-based assessment tool that the Registered Nurse can use within the context of your case study. Explain its significance. 4. Include the use of at least one patient-care technology and explain the benefits to improving health and quality of life of the older adult from your case study. 5. Select at least one appropriate healthcare policy or health program and explain how it is beneficial in the care of the older adult from your case study. 6. Describe your professional nursing role in health promotion and wellness of the older adult identified with the geriatric syndrome from your case study.
